Insurance based PT Policies

• A PT script is required for evaluation, this is for the patient's benefit to ensure insurance coverage

• A patient evaluation is required prior to any treatment for each injury or complaint.

• Length and content of PT/OT session is based upon therapist recommendation following PT/OT evaluation.

• Treatment plan will be discussed with patient prior to any delivery of service or charge.

• Treatment is fee for service unless your insurance is accepted or will reimburse charges.
