Dr. Sara Hulseberg, DPT, MSPT, CSCS, CSYI

EXCEL Therapy & Conditioning is owned and operated by Dr. Sara Hulseberg. Dr. Hulseberg is a graduate of Minnechaug Regional High School. She completed her Bachelor's and Master's Degrees in Physical Therapy locally at Springfield College and went on to obtain her Doctorate from Northeastern University. She was honored to be chosen for Business West's Class of 2023's 40 Under 40Springfield Symphony Hall's Fearless Women's Class of 2025, and countless "Best Of" categories across the region. She has been a professor at Springfield College, a mentor with the Level Up Initiative, an administrator of the #P4520 Foundation, and continues to speak at various events and is a clinical instructor for several universities and programs across the country. She regularly volunteers her time and services to local charities and organizations including Rick's Place and Baystate Children's.

She is practiced in various fields of Physical Therapy; with a focus in sports injury, orthopedic, and manual therapy. Dr. Hulseberg is also a Certified Strength and Conditioning Specialist, making her uniquely qualified in the treatment and strength training of athletes; from novices, to weekend warriors, to D-1, and Olympic level athletes. She is also a certified Travel Agent, sings with various bands, and is a proud mother to two sons.

David Machcuong Ho, NP, RN, BSN, MSN-FNP

David Machcuong Ho, a highly accomplished and passionate Dual Board Certified Nurse Practitioner (NP). David's educational journey commenced in 2013 with an internship at Tufts University School of Medicine, where he discovered his love for beauty, art, science, and medicine. He earned his Bachelor’s of Science in Nursing (BSN) from Elms College School of Nursing, followed by his Master of Science in Nursing (MSN-FNP) with a concentration in Family Nurse Practitioner at Chamberlain University School of Nursing.

Prior to joining Revive and EXCEL, David garnered eight years of valuable experience at Baystate Health, excelling as a phlebotomist and Perianesthesia Registered Nurse (RN). He is a proud member of both the American Association of Nurse Practitioners (AANP) and the American Nurses Association (ANA).

As an ambitious and dedicated nurse practitioner, David finds deep fulfillment in providing exceptional care to others.

Beyond his professional endeavors, David cherishes spending quality time with his partner and pug, Rosie.

Tricia Kirk, CSCS, BS

Patricia graduated from Alfred University with a BS in Health and Fitness Management and a minor in Biology. She has been with EXCEL and trained under Dr Hulseberg since 2022, obtaining her CSCS in 2025. Making her uniquely qualified to fill the gaps between where insurance covered therapy ends, and return to sport/work/life training begins. She is an accomplished equestrian in both English and Western disciplines.

Len Haggerty CSCS, MS, TPI L.1

Coach Haggerty, developer of the MOVE.TRAIN.PLAY. training system is a Certified Strength and Conditioning Specialist (CSCS) with extensive experience helping athletes improve performance, reduce injury risk, and reach their full potential. Len holds master's degrees in Exercise Physiology & Performance and Sports Performance Data Analytics. He is also certified through the Titleist Performance Institute (TPI Level 1) and has extensive experience utilizing the Functional Movement Screen (FMS) and other movement assessment tools to identify limitations and opportunities for improvement. Throughout his career, he has worked with collegiate athletes at all levels, including NCAA Division I football, basketball, and softball programs, as well as professional athletes competing in organizations such as the NBA, PGA, MLL/PLL, and Team USA. His diverse background working with the general adult population and youth athletes across multiple sports, positions, genders, and competitive levels allows him to identify the physical qualities and performance indicators most critical to success. Combining his expertise in strength and conditioning with advanced training in sport performance data analytics, Len helps his clients, athletes, coaches, and support staff make informed decisions that enhance performance, improve health, and reduce injury risk.

When he is not coaching or studying performance, Len enjoys golfing, snowboarding, playing the mandolin, and spending time with his family.

John Durham, Office Manager

JD is a Certified Running Coach and has over 30 years of running experience, specializing in half and full marathons. His passion for the sport is evident in his record of achievements, including qualifying for the Boston Marathon an impressive 5 times. As a coach JD particularly enjoys honing runners ability to find and control their pace and fuel economy. He coaches through mentorship in order to help his runners to achieve their unique goals. JD is also the Office Manager and takes pride in keeping EXCEL running smoothly.

Justin Howard, CSCS, BS, c-BFR

Justin Howard is a Certified Strength and Conditioning Specialist with a Bachelor's degree in Exercise Science from American International College. He is currently pursuing a Master's degree in Clinical Exercise Physiology at Springfield College. He also holds a certification in BFR Training. Known for his calm demeanor, Justin creates a warm and inviting experience where clients feel supported, engaged, and enjoy the process.
