Cash/Out-of-Pocket Therapy

As per insurance contracts, if a patient does not report an active insurance, they are allowed to be provided a cash-based or out-of-pocket alternative utilizing the same fee schedule charged to insurance companies, and available at a discount to the patient when paid upon check-in for an appointment using cash or cash alternative (debit card) options.

Cash based Physical Therapy: $100 per visit

PT Policies:

• A patient evaluation is required prior to any treatment for each injury or complaint.

• Length and content of PT/OT session is based upon therapist recommendation following PT/OT evaluation.

• Treatment plan will be discussed with patient prior to any delivery of service or charge.

• Out-of-Pocket fees will be discounted when paid in CASH or DEBIT and AT TIME OF SERVICE.
